Varicose veins are enlarged, bulging and tortuous veins under the skin. Any vein may become varicose, but the veins most commonly affected are those in the legs and feet. It is because standing and walking upright increases the pressure in the veins of legs & feet.

Pathophysiology

There are two main systems of veins in the leg.

Superficial venous system: Present under the skin & can form varicose veins.

Deep venous system: The leg muscles squeeze the deep veins during walking, carrying most of the blood from legs towards the heart.

Both superficial & deep veins are connected with each other through multiple small channels called perforators. Normally, blood flows from superficial to deep veins through these perforators. Reverse blood flow in perforators leads to formation of varicose veins. Also, reflux in sapheno-femoral junction (SFJ) and sapheno-popliteal junction (SPJ) adds to increased pressure in superficial veins and causes varicose veins. All of these veins contain one-way valves to ensure that the blood flows towards the heart. Failure of these valves allows blood to flow backwards down the veins and results in an overload of pressure when standing. This excess pressure leads to blood flow back into super facial veins and causes varicose veins to develop.

Risk Factors

  • Professions with Standing or sitting for long periods of time in the same position.
  • Pregnancy
  • Family history of Varicose Veins
  • Obesity
  • Female sex – Hormonal factors including puberty, pregnancy, menopause, oral contraceptive pill, Hormone replacement therapy etc affect the disease.

Signs & Symptoms

Symptomatic varicose veins may present with following signs & symptoms.

  • Aching or heavy feeling in the legs especially after exercise or at night.
  • Burning, throbbing, muscle cramping and swelling in the lower legs
  • Worsened pain after sitting or standing for a long time Itching around the veins with dry and red skin (venous eczema)
  • Bleeding from varicose veins ulcer
  • Painful cord like vein with red discoloration of the skin (Thrombophlebitis)
  • Black, thickened & wrinkled skin in near ankle (Lipodermatosclerosis)

A symptomatic varicose veins may present with following signs:

  • Veins that are dark purple or blue in color
  • Visible bulging, tortuous veins

Complications

Complications of varicose veins, although rare, can include:

  • Ulcers: Painful, non-healing, particularly near the ankles.
  • Thrombophlebitis: Painful, cord like veins with skin discoloration.
  • Bleeding: Occasionally, veins near the surface of skin may bleed if you cut or lump your leg

Diagnosis

Venous colour Doppler to diagnose and identify the level of reflux in veins.

Self-Care

This can help you ease the pain of varicose veins and may prevent them from getting worse.

  • Walking, exercises reduce the weight, if obese
  • Avoid standing/sitting for longer period of time in same position
  • Avoid alcohol
  • Do not cross the legs while sitting
  • void tight clothing
  • Elevating the legs
  • Wearing grade II compression stockings, from foot to upper thigh level

When to Consult Doctor

  • Varicose veins are painful.
  • They get worse or do not improve with self-care, such as by wearing compression stockings or avoiding standing or sitting for too long.
  • Develop sudden increase in pain, swelling, fever, redness of leg, or leg ulcer.
  • Develop leg ulcer that does not heal.

Endovenous Laser Treatment (EVLT)

Endovenous laser treatment (EVLT) is the best available treatment for varicose veins, which is now available at SRMS Institute of Medical Sciences, Bareilly.

Endovenous (or vein) ablation is a minimally invasive, outpatient treatment for venous insufficiency/varicose veins that is performed under imaging guidance. After applying local anesthetic to the skin over the vein, an interventional radiologist inserts a thin tube known as a catheter, into the abnormal saphenous vein. Through this catheter, laser energy is applied inside the vein to close it.

By closing the abnormal vein, the varicose veins-which are close to the skin-shrink and improve in appearance. In addition, once the diseased vein is closed, the Other healthy veins take over to carry blood from the leg, re-establishing normal flow.
