A 66-year-old man with chronic liver disease (ethanol + HBsAg related) came to SRMS Institute of Medical Sciences (IMS), Bareilly after months of recurrent, debilitating anaemia. Multiple admissions, repeated transfusions, countless visits, yet no clear source of bleeding, was a major challenge. CT abdomen scans, Upper GI Endoscopy, and Colonoscopy revealed little beyond a benign colonic polyp that didn’t explain the severity of his condition.
Instead of stopping there, our clinicians at SRMS questioned the mismatch between the colonoscopy findings and clinical severity. However, choosing clinical wisdom over convenience, the team, led by Dr Shivam Gupta, Consultant, Department of Gastroenterology at SRMS IMS, pursued and identified a mid-gut evaluation and performed video Capsule Endoscopy that became the turning point, revealing multiple actively bleeding Angio ectasias in the proximal jejunum. This confirmed the small bowel as the source, with the colonic polyp serving as an incidental finding. What seemed like a dead end became a life-changing diagnosis, allowing focused treatment and sparing the patient from unnecessary procedures and interventions.
